Evista (Raloxifene)
DRUG CLASS AND MECHANISM : Estrogen is a hormone which have an ability to regulate the turnover (formation and destruction) of bone (amoung other actions). In case of menopause or after removal of the ovaries, the levels of estrogen are decreased. This situation can cause to osteoporosis, the disease when the bone lose it's density and become weakened. Raloxifene reduses destruction of the bone and extends bone density, but certainly not so good as natural estrogen. This makes bones stronger and averts fractures in women suffering from osteoporosis. Raloxifene is called a "selective estrogen receptor modulator" as it has estrogen effects on some tissues but blocks the effects of estrogen on other tissues. Raloxifene reduses low density lipoprotein (LDL or "bad") cholesterol in the blood, but unlike estrogen, raloxifene does not extend high density lipoprotein (HDL or "good") cholesterol.

SIDE EFFECTS : This drug may cause dizziness, lightheadedness, headache, stomach upset, bloating, or nausea.
These effects should disappear as your body get used to the medicine. Inform quickly: mental/mood changes, skin rash, hot flashes,
stomach pain, breast tenderness, blurred vision, abnormal vaginal bleeding. If you reveal other effects not listed above, tell
about it to your doctor or pharmacist.
PREPARATIONS : Tablets, 60mg.
STORAGE : Tablets should be kept between 15° (59°F) and 30°C (86°F).
PRESCRIBED FOR : Raloxifene is applied for the prevention and treatment of osteoporosis in post-menopausal women.
DOSING : Raloxifene is usually accepted once a day. It can be taken with food or without it.
Persons may need lower doses in case of cirrhosis.
DRUG INTERACTIONS : Cholestyramine (Questran) decreases the absorption of raloxifene, that's why these two drugs
should be accepted with a difference of several hours. Raloxifene may slightly decrease the ability of blood to thrombosis and
thus extend the effects of thrombolitic medicines. That's why in case of combination of raloxifene with blood thinners such as
warfarin (Coumadin), the ability of blood to clot may need to be investigated more closely with frequent measurements of the
prothrombin time of blood.
PREGNANCY : Estrogens (and probably raloxifene) shouldn't be accepted during pregnancy because of the
unknown action on fetus, although there is an increased risk of fetal abnormalities with the use of estrogens.
NURSING MOTHERS : Raloxifene should not be advised to nursing mothers.
SIDE EFFECTS : Raloxifene has few collateral effects. The most common are hot flashes (seen in 1 of every four
persons), sinusitis (1 in 10), weight gain (1 in 11), muscle pain (1 in 12), leg cramps (1 in 20), and ankle swelling (1 in 30).
Raloxifene extends the risk of blood clots, including deep vein thrombosis (DVT) and pulmonary embolism (blood clots in the lung).
The most dangerous for development of complications are the first 4 months of usage. Patients taking raloxifene should avoid long
periods of restricted movement during travel when the risk of blood clots is very high.
