DRUG CLASS AND MECHANISM : It has been established that 140 million men worldwide suffered from impotence

Sildenafil also decreases the pressure in the pulmonary artery in a serious condition called pulmonary arterial hypertension.
By blocking an enzyme called phosphodiesterase-5 (PDE5) which destroys the cGMP sildenafil averts the destruction of cGMP and
allows cGMP to accumulate and persist longer. This situation leads to more prolonged engorgement of the penis.

DOSING : Sildenafil is rapidly absorbed. The maximum concentrations are reached within 30 to 120 minutes (median 60 minutes) of oral dosing in the fasted state. DRUG INTERACTIONS : Sildenafil extends the effects of the blood pressure lowering medications including nitrates, e.g. isosorbide dinitrate (Isordil), isosorbide mononitrate (Imdur, Ismo, Monoket), nitroglycerin (Nitro-Dur, Transderm-Nitro) that are accepted primarily for treating angina. Patients taking nitrates shouldn't consume sildenafil. Cimetidine (Tagamet), erythromycin, ketoconazole (Nizoral), itraconazole (Sporanox) and mibefradil (Posicor) can lead to enlarge sildenafil in the body. In case of using these drugs patients should be observed carefully if sildenafil is accepted. It is known that rifampin can reduse blood levels of sildenafil and probably diminish its effectiveness.
PREGNANCY : Although tests carried out in animals haven't demonstrated negative effects on the fetus, sildenafil hasn't been studied in pregnant women. There is no effect on sperm quantity or mobility of sperm in men.
NURSING MOTHERS : It is unknown whether sildenafil is excreted into breast milk.
SIDE EFFECTS : Over 15% of patients accepting sildenafil experience side effects. The most common of them are facial flushing (1 in 10), headaches (1 in 6), stomach pain, nasal congestion, nausea, diarrhea, and an inability to differentiate between the colors green and blue.
