Lamisil (Terbinafine)
DRUG CLASS AND MECHANISM : Terbinafine is an antifungal agent that is accepted orally or applied to the skin. Terbinafine operates on the ability of fungi to produce chemical compounds called sterols. This compaunds are an important part of the membrane that surrounds fungal cells and holds them together. These operations weakens the cell membrane. Oral terbinafine is more effective for treatment of fungal nail infections than griseofulvin (Fulvicin; Gris-Peg) and itraconazole (Sporanox), though two other antifungal medications are also used for treating fungal nail infections. Topical terbinafine was approved by the FDA in 1993. Terbinafine oral tablets were approved in 1996.

PREPARATIONS : Tablets containing 250mg. Cream containing 1% terbinafine, 15gm and 30gm size tubes.
STORAGE : The tablets and the cream should be stored at room temperature, 15-30°C (59-86°F).
PRESCRIBED FOR : The tablets of terbinafine are used for treatingf ungal infections of the toe, or fingernail,
caused by the fungus, tinea unguium. Cream is appointed in case of fungal infections of the skin including tinea pedis
("athlete's foot"), tinea corporis, and tinea cruris ("jock itch").
DOSING : Tablets: The advised dose of terbinafine for treatment of the fingernails is 250 mg once a day for 6 weeks, and 12 weeks for treatment of toenails. Other method allows to give terbinafine in a mode of two tablets (500 mg) once a day for 7 days during the first week of each month for 3 months for toenail infections. For children under 20kg (44 pounds) the advised dose is 62.5 mg/day and for children between 20 and 40kg (44 and 88 pounds) a dose of 125 mg/day have been recommended. Terbinafine can be accepted with meals or without them. Cream: The cream applied twice a day on the affected area. The rate is usually lasted from 1 till 4 months.
DRUG INTERACTIONS : Rifampin decreases blood's concentration of terbinafine causing decline of efficiency of terbinafine. Cimetidine (Tagamet) may extend terbinafine blood levels but this effect wouldn't lead to problems.
PREGNANCY : The manufacturer of terbinafine does not recommend therapy during pregnancy as so as fungal infections of the skin and nails usually are not a serious problem and there have been no studies in humans. Studies in animals using large dosages of terbinafine have not shown toxic effects on the fetus.
NURSING MOTHERS : There were no reports about action of terbinafine in breast-feeding women. Nursing mothers shouldn't use terbinafine on the breast or taking the drug orally.
SIDE EFFECTS : Terbinafine is a very well tolerated and safe medicine. The necessity in stopping therapy because of side effects is quite rare. The most frequently reported side effects are diarrhea, and abdominal pain. There have been reports about extending in liver enzymes, hives, itching and altered taste sensation.
