Prilosec, Rapinex (Omeprazole)
DRUG CLASS AND MECHANISM : Omeprazole is in a class of drugs called proton pump inhibitors (PPI). It acts by inhibiting the production of acid by the stomach. Other drugs of this class are lansoprazole (Prevacid), rabeprazole (Aciphex), pantoprazole (Protonix), and esomeprazole (Nexium). Proton pump inhibitors are appointed for the treatment of conditions caused by acid such as stomach and duodenal ulcers, gastroesophageal reflux disease (GERD) and the Zollinger-Ellison syndrome. Omeprazole, like other proton-pump inhibitors, blocks the enzyme in the wall of the stomach that produces acid. By stopping the production of enzyme, the secretion of acid into the stomach becomes low and this process gives an opportunity for the stomach and esophagus to heal. Omeprazole OTC has been approved for sale without a prescription.

PREPARATIONS : Omeprazole by prescription: 10, 20 & 40 mg. Omeprazole OTC: 20 mg tablet.
Omeprazole powder for oral suspension (Rapinex), which is rapidly absorbed, is released in 20 mg packets for
dilution with water. It is used in combination with sodium bicarbonate to neutralize stomach acid that would destroy the omeprazole.
STORAGE : Omeprazole should be stored at room temperature, 15-30°C (59-86°F). Keep away from moisture and light.
PRESCRIBED FOR : Omeprazole is appointed for the treatment of acid-induced inflammation and ulcers of the stomach
and duodenum, gastroesophageal reflux disease (GERD) and Zollinger-Ellison Syndrome. It also is applied in combination with antibiotics
for eradicating H. pylori infection of the stomach.
DOSING : For ulcers, GERD and eradication of H. pylori the advised dose for adults is 20-40 mg daily. Ulcer healing usually appears within 4-8 weeks. H. pylori infections are treated for 10-28 days. Omeprazole OTC was approved for more severe heartburn for up to two weeks. For the regulation of Zollinger-Ellison Syndrome the advised starting dose for adults is 60 mg a day, and the dose is adapted based on either the response of symptoms or the actual regulation of acid production. Doses greater than 80 mg should be divided. Doses up to 120 mg three times a day have been appointed in the treatment of Zollinger-Ellison Syndrome. For maximal efficacy, omeprazole tablets should be accepted before meals, swallowed whole and shouldn't be crushed, chewed or opened.
DRUG INTERACTIONS : Omeprazole potentially can extend the concentrations in blood of diazepam (Valium), warfarin (Coumadin), and phenytoin (Dilantin) by redusing the elimination of these drugs by the liver. The absorption of certain drugs may be affected by stomach acidity, and, as a result, omeprazole and other PPIs may bring down stomach the absorption and concentration in blood of ketoconazole (Nizoral) and extend the absorption and concentration in blood of digoxin (Lanoxin). his may cause the reduction of effectiveness of ketoconazole or increasement digoxin toxicity, respectively.
PREGNANCY : Use of omeprazole in pregnant women has not been apprised.
NURSING MOTHERS : The influence of omeprazole has not been studied in nursing women.
SIDE EFFECTS : Omeprazole like other PPIs is a well-tolerated drug. The most common side effects are diarrhea, nausea, vomiting, headaches, rash and dizziness. Nervousness, abnormal heartbeat, muscle pain, weakness, leg cramps and water retention occur infrequently. Each dose of omeprazole powder for oral suspension contains 460 mg of sodium, and this should be accepted into consideration in patients who need a sodium restricted diet.
