DRUG CLASS AND MECHANISM : Metformin is a drug that should be accepted orally. It reduces blood glucose (sugar), and that's why it is applied for the treatment of type 2 diabetes. Insulin is a hormone produced by the pancreas. It causes a decline of glucose blood's levels by decreasing the quantity of glucose produced by the liver and it is also extends the removal of glucose from the blood by muscle and fat tissues. The reason of a diabetes is a decline in production of insulin and lowering uptake (and effects) of insulin on the body's tissues. Metformin decreases the level of blood's sugar by extending the sensitivity of liver, muscle, fat, and other tissues to the uptake and effects of insulin. At single application metformin doesn't lead to sharp reduction in sugar of blood as doesn't increase the content of insulin in blood, as against glucose-lowering drugs of the sulfonylurea class, for example, glyburide (Micronase; Diabeta) or glipizide (Glucotrol). It was reported that metformin decreased the complications of diabetes such as heart disease, blindness and kidney disease. Metformin was approved by the FDA in December of 1994.

PREPARATIONS : Metformin tablets: 500, 850, and 1000 mg. Glucophage XR (extended release) tablets: 500 and 750 mg. Glumetza tablets(extended release) 500 and 1000 mg.
STORAGE : Keep at room temperature between 20-25°C (68-77°F).
PRESCRIBED FOR : Metformin is appointed for the treatment of type II diabetes in adults and children. It may be accepted alone or combined with other diabetic drugs. Metformin also has been applied for averting the development of diabetes in people having risk for diabetes and to treat polycystic ovaries.
DOSING : At the begining the advised dose of metformin for adults is 500 mg twice a day or 850 mg once daily. The dose is gradually extended by 500 mg weekly or 850 mg every two weeks as tolerated and based on the response of the levels of glucose in the blood. The maximum daily dose is 2550 mg which is broken into three times within day. If Glucophage XR tablets are appointed, the patient should start from the dose is 500 mg daily with the evening meal. The dose can be extended by 500 mg weekly up to a maximum dose of 2000 mg once a day or in two divided doses. Glumetza tablets are accepted once daily. Metformin should be taken with food. The starting dose for pediatric patients of 10-16 years is 500 mg twice a day. Dosage can be extends by 500 mg weekly up to a maximum dose of 2000 mg. It is not known about action of Glucophage XR in children.
DRUG INTERACTIONS : Cimetidine (Tagamet) may extend the quantity of metformin in the blood by 40% because it reduses the elimination of metformin from the body and this can cause the enlarge of side effects from metformin.
PREGNANCY : There have been no researches in pregnant women. The majority of researchers believe that insulin is the best treatment for pregnant women with diabetes.
NURSING MOTHERS : Metformin is excreted into breast milk and can penetrate the nursing infant. Nursing women shouldn't accept metformin.
SIDE EFFECTS AND PRECAUTIONS : Metformin has few collateral effects.The most common are nausea, vomiting, gas, bloating, diarrhea and loss of appetite. These symptoms appear in one out of every three patients. These by-effects can be expressed enough and may cause to a cancellation of therapy in one out of every 20 patients. These side effects are related to the dose of the medication and may diminish if the dose is lowered. The most serious side effect of metformin is lactic acidosis appears in one out of every 30,000 patients and is fatal in 50% of cases. This side effect is occured rarely. The symptoms of lactic acidosis are weakness, trouble breathing, abnormal heartbeats, unusual muscle pain, stomach discomfort, light-headedness and feeling cold. Patients reduced function of the kidneys or liver, congestive heart failure, severe acute illnesses, and dehydration are at risk for lactic acidosis.
