Amaryl (Glimepiride)
DRUG CLASS AND MECHANISM : Glimepiride belongs to a group medicines called sulfonylureas and is used to control diabetes by redusing blood sugar. It should be accepted orally. Other drugs of this clsss are glyburide (Micronase; Diabeta), glipizide (Glucotrol), tolbutamide (Orinase) and tolazamide (Tolinase). Glimepiride is applied for treatment of the most usual type of diabetes type II diabetes. IT is found in 90% of patients with diabetes. At type II diabetes, there is no necessity to operate blood's sugar with the help of insulin. It is enough to keep to a diet and accept oral preparations. Immunity to sugar which consists in increase in a level of blood's sugar is caused by reduction of secretion of insulin in a pancreas and stability of cells of a body to influence of insulin. Glimepiride stimulates secretion of insulin by a pancreas which promotes penetration of sugar into cells of a body, thus, lowering sugar of blood. Glimepiride was approved by the FDA in December of 1995.

PRESCRIBED FOR : At a diabetes the control of sugar of blood is necessary. It helps to reduce an opportunity of damage to eyes, kidneys and nerves . At type II diabetes when it is not enough only a diet, glimepiride should be appointed. In case of absence of effect other oral medicine or insulin can be used.
DOSING : As the doze is selected individually, it is necessary to carry out periodic measurements of sugar, for selection of the most suitable dose like at treatment by other medicines from this group.
DRUG INTERACTIONS : Increasing blood sugar drugs can change the action of glimepiride. These medicines are diuretics [e.g. hydrochlorothiazide, furosemide (Lasix)], corticosteroids such as prednisone and methylprednisolone (Medrol)], phenytoin (Dilantin), niacin, and sympathomimetics such as pseudoephedrine (Sudafed). Beta-blockers such as propranolol (Inderal) and atenolol (Tenormin) can change blood sugar, to increase or reduce it. Also they can weaken the sugar-lowering effect sulfonylureas and make it less productive. Beta-blockers can blunt some reactions of an organism on reduction of blood's sugar, thus prevent patients to understand when sugar is low. Even having such effect beta-blockers are successfully used for treatment diabetic patients because of good survival in diabetics with high blood pressure. Rifampin accelerates transformation of two other sulfonylureas, tolbutamide (Orinase) and glyburide (Diabeta, Micronase). This might interfere with the effects of the medicines and lead to higher levels of sugar in the blood. Although there were no messages about such interference with glimepiride, the opportunity of it should be taken into account.
PREGNANCY : SGlimepiride and other drugs of this group increased risk of fetal death in animal studies. Studies in women havn't been spent. On the other hand, high or low blood sugar concentrations during pregnancy increase the risk of abnormalities in the fetus that's why docktor must carefully weigh the advantage and risks of using this drug during pregnancy.
NURSING MOTHERS : It is unknown if glimepiride is excreted in breast milk like other sulfonylureas or not. Glimepiride should not be accepted by breast-feeding women because of the risk of low blood sugar in the infant. If women needs therapy exept diet and exercise there should be recommended insulin.
SIDE EFFECTS AND PRECAUTIONS : Glimepiride can lead to low blood sugar which symptoms include hunger, nausea, tiredness, perspiration, headache, heart palpitations, numbness around the mouth, tingling in the fingers, tremors, muscle weakness, blurred vision, cold temperature, excessive yawning, irritability, confusion, or loss of consciousness. Another side effects are dizziness (1 of every 60 persons), headache (1 of every 75), and nausea or vomiting (1 of every 90). A rash occurs in fewer than 1 of every 100 persons who accept glimepiride.
