DRUG CLASS AND MECHANISM : Simvastatin is a cholesterol- lowering medicine acts by blocking the production of cholesterol by the liver. It reduse total blood cholesterol as well as LDL cholesterol levels. LDL cholesterol is known as "bad" cholesterol that is primarily responsible for the development of coronary artery disease. Lowering LDL cholesterol levels can slow down the progression and may even reverse coronary artery disease.

PRESCRIBED FOR : The treatment of high blood cholesterol begins with exercise, weight loss, and a diet low in cholesterol and saturated fats. If these measures don't help in achievement enough cholesterol-lowering, medications such as simvastatin may be added. The National Cholesterol Education Program (NCEP) has published approaches to treatment with the help of these medications which take into account the level of LDL cholesterol as well as the presence of other factors that extend the risk for coronary artery disease such as diabetes, hypertension, cigarette smoking, low HDL cholesterol level, and family history of early coronary heart disease. The effectiveness of the medication in lowering cholesterol depends on a dose. It is necessary to investigate blood cholesterol levels regularly for making adjustments in doses.

DOSING : TMay be accepted with meals or without them.

DRUG INTERACTIONS : Simvastatin is generally a well- tolerated drug. Simvastatin should be appointed cautiously in patients with alcohol or other liver diseases. Persistently abnormal liver tests during treatment are rare, but may lead to a discontinuation of the drug. There were reports about muscle inflammation (myositis) and breakdown in case of using simvastatin. Muscle breakdown causes the release of muscle protein (myoglobin) into the blood and kidney tubules. This can result in acute kidney failure. The risk of muscle breakdown is extended when simvastatin is appointed together with other medications such as cyclosporine (Sandimmune), gemfibrozil (Lopid), erythromycin and nicotinic acid. Simvastatin may interact with cholestyramine (Questran), warfarin (Coumadin), and cimetidine (Tagamet) to change the blood levels of these drugs. It is necessary to research blood clotting times for preventing excessive blood thinning and bleeding in case of using Coumadin in combination with simvastatin. Simvastatin should not be applied in children. Simvastatin is not habit forming.

PREGNANCY : Simvastatin should not be accepted by pregnant women.

NURSING MOTHERS : Simvastatin should not be used by nursing mothers because of potential adverse side effects to the nursing infant.

SIDE EFFECTS : Simvastatin is generally a well-tolerated drug and side effects are rare. Slight side effects include constipation, diarrhea, fatigue, gas, heartburn, and headache. Serios side effects include abdominal pain or cramps, blurred vision, dizziness, easy bruising or bleeding, itching, muscle pain or cramps, rash, and yellowing of the skin or eyes.
