Lipitor (Atorvastatin)

DRUG CLASS AND MECHANISM : Atorvastatin is an oral medicine acts by lowering the level of cholesterol in the blood. It belongs to a class of medicines called statins which avert the production of cholesterol by the liver. They block HMGCoA reductase, the enzyme that makes cholesterol. Other drugs of this group are lovastatin (Mevacor), simvastatin, (Zocor), fluvastatin (Lescol), and pravastatin (Pravachol). They reduse total blood cholesterol as well as LDL cholesterol levels. (LDL cholesterol is known as "bad" cholesterol that is primarily responsible for the development of coronary artery disease.) Lowering LDL cholesterol levels can slow down the progression and may even reverse coronary artery disease. Unlike the other drugs in this class, atorvastatin also can decrease the concentration of blood's triglycerides. High blood concentrations of triglycerides also have been associated with coronary artery disease. Atorvastatin was approved by the FDA in December of 1996.

PRESCRIBED FOR : Atorvastatin is appointed for the treating high cholesterol and triglyceride levels. The treatment of high blood cholesterol begins with exercise, weight loss, and a diet low in cholesterol and saturated fats. If these measures don't help in achievement enough cholesterol-lowering, medications such as atorvastatin may be added. The National Cholesterol Education Program (NCEP) has published approaches to treatment with the help of these medications which take into account the level of LDL cholesterol as well as the presence of other factors that extend the risk for coronary artery disease such as diabetes, hypertension, cigarette smoking, low HDL cholesterol level, and family history of early coronary heart disease. The effectiveness of atorvastatin in lowering cholesterol depends on a dose, that means that higher doses decrease cholesterol in greater degree. It is necessary to investigate blood cholesterol levels regularly for making adjustments in doses.

DOSING : Atorvastatin is appointed once a day. The advaised starting doses are 10-40 mg. daily. It may be taken with meals or without them and at any time of day.

DRUG INTERACTIONS : As with other drugs in this class, the risk of muscle breakdown (see "Side Effects," below) is increased when atorvastatin is given together with other medications such as cyclosporine (Sandimmune), gemfibrozil (Lopid), erythromycin and nicotinic acid.

PREGNANCY : There were no reports about affection of atorvastatin in the fetus that's why atorvastatin may be used in pregnancy if the physician feels that its benefits outweigh the potential risks.

NURSING MOTHERS : It is unknown if atorvastatin is secreted in breast milk, that's why the doctor must take a decision about continuation of taking atorvastatin in pregnant and nursing woman. It depends from the benefits of treatment and the potential risks.

SIDE EFFECTS : AAtorvastatin is a well-tolerated drug. Some rare side effects include constipation, diarrhea, fatigue, gas, heartburn, and headache. Atorvastatin should be appointed cautiously in patients with alcohol or other liver diseases. Persistently abnormal liver tests during treatment are rare but may lead to discontinuation of the drug. There were reports about muscle inflammation (myositis) and breakdown in case of using other drugs in this class (HMGCoA reductase antagonists), and it is assumed that this side effect also may appear with atorvastatin. Muscle breakdown results in the release of muscle protein (myoglobin) into the blood and accumulation of the protein in the kidney tubules, leading to kidney failure.
