Cozaar (Losartan)
DRUG CLASS AND MECHANISM : Losartan and its primary metabolite block the receptor of angiotensin which was found in many tissues, primarily in vascular smooth muscle. Angiotensin, formed under action of angiotensin converting enzyme (ACE), is a powerful chemical compaund leads to narrowing of blood vessel (vasoconstriction) which can cause the elevation of blood pressure (hypertension).

PREPARATIONS : Cozaar 25 and 50 mg film coated tablets.
STORAGE : Tablets should be kept at room temperature in a tightly closed, light resistant container.
PRESCRIBED FOR : Losartan is accepted for the treatment of hypertension. It may be used alone or in combination with other agents. It can be less effective in black patients, related to ACE inhibitors, such as captopril (Capoten), enalapril (Vasotec), and lisinopril (Zestril).
DOSING : Losartan can be taken with meals or without them. Losartan is breaked in the liver under action of cytochrome P450 enzymes to an active metabolite that is responsible for most of the drug activity. Decline of dose in 50% is recommended in patients with broken function of liver .
DRUG INTERACTIONS : Losartan must be accepted autiously in patients with depleted volume caused by excessive decline in blood pressure after use. Losartan may worsen kidney function in patients dependent on angiotensin activity to maintain kidney blood flow, related to ACE inhibitors. It sometimes is visible at diabetics with worsened kidney function and in patients with narrowings in one of the main arteries of kidney. There were no applications for the important medicinal interactions, but inhibitors of cytochrome P450 such as ketoconazole (Nizoral) have been shown in lab studies to block the formation of the active drug metabolite, that's why losartan should be used cautiontly in patients taking Nizoral, as it could be expected the decline of losartan activity. There were no reports about losartan's safety and efficacy in children.
PREGNANCY : Losartan and other related drugs can cause injury and even death to the fetus, that's why losartan should not be accepted during pregnancy. When pregnancy is detected, losartan should be stopped as soon as possible.
NURSING MOTHERS : Losartan penetrates to rat milk, and it is toxic to rat neonates. Therefore, it should be canceled in nursing mothers.
SIDE EFFECTS : In studies of over 4000 patients, including 1200 treated for over 6 months and 800 for over 1 year, displays of side effects were similar to placebo. Losartan is generally well tolerated. Side effects ara common and includ diarrhea, muscle cramps, dizziness, insomnia, and nasal congestion.
