Zithromax, Zmax (Azithromycin)

DRUG CLASS AND MECHANISM : Azithromycin is a semi-synthetic macrolide antibiotic chemically similar to erythromycin and clarithromycin (Biaxin). It acts on a wide variety of bacteria organisms, such as Hemophilus influenzae, Streptococcus pneumoniae, Mycoplasma pneumoniae, Staphylococcus aureus, and mycobacterium avium, and many others. Its advantage in a prolonged persistence in the body, allowing for once a day dosing and for shorter treatment courses for most infections. Like all macrolide antibiotics, azithromycin averts bacteria from growing by interfering with their ability to produce proteins. Due to distinctions in synthesis of fibers in bacteria and humans, the macrolide antibiotics do not interfere with humans' production of proteins.

PREPARATIONS : Capsules (red): 250 mg; Suspension: 100 and 200 mg/teaspoon. Zmax extended release susspension in bottles of two gm to be reconstituted in 60 mL of water.


STORAGE : Capsules should be stored below 30°C (86°F). Suspension should be stored between 5° and 30°C (41° and 86°F).


PRESCRIBED FOR : Azithromycin acts on susceptible bacteria causing infections of the middle ear, tonsillitis, throat infections, laryngitis, bronchitis, pneumonia and sinuses. It is also effective against certain sexually transmitted infectious diseases, such as nongonococcal urethritis and cervicitis.

DOSING : Azithromycin should be accepted one hour before or two hours after meals as it may connect with food and not be absorbed from the intestine. For most infections, azithromycin is appointed once a day for a relatively short course of treatment (usually five days). The first dose is often a "double dose," twice as much as the rest doses applied. In case of acute bacterial sinusitis, azithromycin may be accepted once a day for three days. Zmax usually is appointed as a single 2 gm dose.

DRUG INTERACTIONS : As against erythromycin and clarithromycin, azithromycin is considered to be free from interactions with most other drugs. Azithromycin shouldn't be appointed at the same time as aluminum- or magnesium- based antacids, such as Mylanta or Maalox because antacids will connect with azithromycin and avert its absorbtion from the intestine.

PREGNANCY : There have been no adequate studies of azithromycin in pregnant women. Studies in animals suggest no important effects on the fetus. That's why azithromycin can be appointed in pregnant women if the physician feels that it is necessary.

NURSING MOTHERS : It is unknown if azithromycin has secretion in breast milk.

SIDE EFFECTS : Azithromycin is generally a well tolerated drug. The most common side effects are diarrhea or loose stools, nausea, abdominal pain, and vomiting, each of which may appear in fewer than one in twenty persons who consume azithromycin. Rarer side effects include abnormal liver tests, allergic reactions, and nervousness.
